Accident analysis is one of the key aspects in conducting a successful accident investigation. The ability to conduct a methodical and structured analysis, based on a thorough understanding of the approach, is one of the core skills of an accident investigator.


At a glance

What you will learn

The course will cover selected accident analysis techniques and their underlying theories. Factors to be considered in selecting analysis techniques will also be discussed; this includes system structures, system component relationships, system behaviour, trade-offs, and limitations of each approach. Attendees will be encouraged to critically evaluate selected techniques  based on case studies, and to have practical exercises of some of the techniques, including developing timeline/sequence of events, evaluation of evidence, and identifying safety issues. Further exercise for those attending the course as part of the MSc Safety and Accident Investigation will focus on translating analysis models to written reports.

Core content

The aim of this course is to introduce a selection of accident analysis techniques and their underlying accident causation theories, and provide the approach to selecting the techniques and the opportunity to apply them in a learning environment.

Who should attend

The course is aimed at professionals who are involved in accident/incident investigation with the purpose of improving safety in safety critical domains and high reliability organisations, such as civil and military transport (aviation, rail and marine), healthcare, process industries, and general utilities.
